Announcing CHC's New Medical Patient Web Portal

Patient PortalThe Community Health Center is pleased to annouce the opening of our new secure Patient Web Portal.  This has been made possible due to our new partnership with Athenahealth. Athena provides online Electronic Health Records (EHR) services. Using the portal, CHC patients will be able to:

• Exchange messages with our practice
• Review and pay billing statements
• Request appointments
• Research health topics
• Review Personal Health Information
• Complete and update medical forms
• Update your profile and contact information

Patients please be aware that the patient portal is only for patients being seen in the Medical Department of CHC. Dental patients can not currently use the Portal.

Womens Health Month at CHC: Free Mammography Vouchers Available to Qualified Women in Cobb and Cherokee Counties

Through generous gifts made by community members, this December, the Community Health Center will be providing free breast exams to complement free mammogram vouchers which have been provided by Wellstar Health System. Women who qualify need only to make an appointment to have a breast exam in order to receive one free mammogram voucher. This voucher may then be presented for one free mammogram at select Wellstar facilities.

All patients must meet the following qualifications to apply:

• Applicants must be women between the ages of 40 and 74.
• Applicants must live in Cobb or Cherokee Counties, specifically in the following zip codes:
  • 30080, 30144, 30060, 30127, 30066, 30067, 30008, 30126, 30101, 30102, 30064, 30168, 30106, 30152, 30188, 30114.
• Applicants total house hold income must be less than $50,000 / year.
• Applicants must be uninsured or have insurance that does not cover mammogram screenings.
• Applicants must not have had a mammogram in the past year.

All applicants must provide proof of household income and proof of residency in the above specified zip codes before an appointment will be scheduled. If you do not live in the above specified zip codes, you will not be able to qualify for the free exam and vouchers.
