Medicare Free Preventative Services: Watch the New Video

Medicare provides many preventative screenings to its beneficiaries for free such as the Welcome to Medicare visit for new Medicare Members, the Annual Wellness Visit which each Medicare Member can receive yearly, and other screenings like prostate and cholesterol screenings.

Medicare recently added a new preventative services video to help get awareness out to seniors. If you have Medicare or have a family member that does, share this page to help increase awareness of these free services.

CHC provides all these free preventative services to Medicare Part B patients. Request your appointment online today.

CHC Announces Grand Opening of New Low Cost X-Ray Diagnostic Center March 27, 2012

Courtesy zirconicussoThe Community Health Center is pleased to announce the opening our new X-Ray Diagnostic Center on March 27, for families in the greater Metro Atlanta area. Initially, we will be offering x-rays one day a week on Tuesdays. As demand increases, we will add more days each week. We are very excited about this new resource and believe that it will be incredibly valuable to our patients and to others in our community. The center will focus on the uninsured, under-insured and self pay patients. To learn more about this amazing resource, please visit the page for our imaging center. To sign up, please fill out and submit our online x-ray patient form and one of our staff will call you to set up your appointment.

CHC Hosts "Take Care of You" Day for Cancer Survivors 03/27

Cured4LifeCHC is pleased to announce a new partnership with the Cured4Life foundation, a fellow nonprofit organization which focuses on "bringing women together to help build and encourage each other while spreading the vital message of breast cancer awareness."

As a result of this new partnership, the Community Health Center will be hosting a "Take Care of You" seminar, put on by Cured4Life free of charge. This seminar is open to women who are survivors of breast cancer and will focus on providing an intimate setting for survivors to bond and share with others. These sessions could include:

• Motivational Speakers
• Licensed Massage Therapist - will conduct relaxing exercise
• Licensed Certified Hair Stylist - will conduct a makeup and hair styling session

Our first seminar will be held on March 27, 2012 from 10:00am - 12:00pm at the Community Health Center in Austell, GA. Space is limited to 20 participants per session. More seminars will be available throughout the year.

To Qualify:

Participants will be referred from CHC's patient body. They will complete an information form and will receive a referral letter to use as their ticket to get into the seminar. If you are not a patient at CHC, but would still like to attend, please call our office to request a referral or send your request through our website on our Event Registration Page here.

Cured4Life has graciously agreed to provide more seminars free of charge to our cancer survivor patients and we will be posting dates for that soon. Tooth Extraction Special: $100 per Eligible Tooth - Download Coupon Here

digitalartIn 2012, we know that the last thing people want to spend money on is dental care. In order to help serve our patients, the Community Health Center will be begin offering tooth extractions for $100 per eligible tooth*.

Before having tooth extractions, each patient must come in for a Comprehensive Oral exam ($25) which includes x-rays and a treatment plan made by dentists on our staff. During the exam , patients can schedule their extractions. Please be aware that this treatment plan has a minimum of 2 office visits, one for the Comprehensive Oral evaluations and one for the extractions. Depending upon the condition of your mouth, you may require more than two visits to complete the extractions. All of this information will be made clear in the treatment plan that patients will receive in the Comprehensive Oral exam. Please call our office at 678-398-6548 for more information and let them know you are interested in this special or if your ready to schedule, click on "Schedule your Medical or Dental Appointment Here Link" at the top on the right to fill out your paperwork online. Next, download and print out your tooth extration coupon to bring with you to your vist. All patients who wish to use this special must bring their printed coupon to their appointment.

*Teeth eligible for extractions will be determined by dentists on staff. If a tooth can not be extracted at CHC, patients will be made aware in the Comprehensive Oral evaluation before they schedule their extractions.
