Appointments

Thank you for choosing the Community Health Center to meet you medical and dental needs. We are happy to serve you and hope that you will choose CHC as your primary care home.

New Patients

New patients seeking services at CHC are encouraged to request an appointment online via our online patient forms. You may also call our offices at 678-398-6548. (Please note, if you choose to fill your paperwork out in the office, please come 20-30 minutes early to finish your intake packet before your appointment.) Please also note, that currently, CHC requires a prepayment to be made when scheduling any appointment. Please have your pre-payment ready when you call the office to schedule your appointment. See the "Fees" section below for more information.

Existing Patients

Existing patients must update their records every new calendar year and/or if your last visit was over 6 months ago from your next scheduled appointment. Fees

CHC charges fees for each of its services depending on the type of service, its depth of involvement, and provider availability. A list of current services offered by CHC and their corresponding fees may be obtained by clicking on the links above or by contacting our office via telephone at 678-398-6548. Please note, our current offering of services depend strictly on our provider availability. We ask that you please be understanding when applying for service, as our offerings can change. 

Prepayments

Every service has a required prepayment amount that must be submitted when making an appointment. This fee must be paid before your appointment can be scheduled and can not be paid on the day service is received. The amount of the prepayment is dependent on which services will be provided to the patient. Patients will be informed of the amount of the prepayment required for their service at the time of making an appointment. We accept cash or credit (visa/Master Card/Discover), no checks. Please note: Your prepayment is nonrefundable. If you are late for your appointment or if you no-show your appointment, your prepayment will not be refunded to you and you will have to make another prepayment in order to schedule another appointment.

Insurance

In an effort to better serve our patients and make our services available to more people in our community, the Community Health Center has begun the process of credentialing with many different health insurance providers. This will allow patients who do have health insurance to use that insurance at CHC to be seen. Below you will find a list of accepted insurance plans as well as ones we are working on. If you do not see your own insurance plan below, please check back regularly to see if it has been added. Please note that patients can always be seen on a self-pay basis.

Medicare - Part B

In January of 2012, the Community Health Center began to take Medicare Part B insurance as a form of payment for medical services rendered at the Center. Now, Medicare beneficiaries with Part B insurance will be able to see a health care provider at CHC and use their Medicare benefits to pay for the services. All co-pays and deductibles mandated by Medicare will be collected but there are also services that Medicare provides to its beneficiaries free of charge such as the "Welcome to Medicare" preventive services visit that seniors who have Medicare can receive free of charge during their first 12 months after enrollment at age 65 or the "Annual Wellness Visit", which is also a preventative health visit that Medicare Part B beneficiaries are entitled to free of charge every 12 months.
