Appointments

Thank you for choosing the Community Health Center to meet you medical and dental needs. We are happy to serve you and hope that you will choose CHC as your primary care home.

New Patients

New patients seeking services at CHC are encouraged to request an appointment online via our online patient forms. You may also call our offices at 678-398-6548. (Please note, if you choose to fill your paperwork out in the office, please come 20-30 minutes early to finish your intake packet before your appointment.) Please also note, that currently, CHC requires a prepayment to be made when scheduling any appointment. Please have your pre-payment ready when you call the office to schedule your appointment. See the "Fees" section below for more information.

Existing Patients

Existing patients must update their records every new calendar year and/or if your last visit was over 6 months ago from your next scheduled appointment. If you have any questions, please click on the "Questions or Comments?" link on the right side of any page on our website, or call our front desk at 678-398-6548.
